Sarah Casalan remembers several clear details from the night of her heart attack<\/a> two years ago: First, she kept thinking she had indigestion from the hamburger she\u2019d made herself for dinner the night before, though it was unusual, considering her \u201ciron stomach.\u201d But then she felt so awful that she lay on the bathroom floor, sweaty and nauseous, for over an hour\u2014and found she could not get up.<\/p>\n

\n

\u201cThat was when the alarm bells went off, though I couldn\u2019t, even at that moment, imagine I was having a heart attack<\/a>,\u201d says Casalan, president of the UPS Store Inc. and a single mom to two boys who were 6 and 7 at the time. After all, she was just 47, active, and in generally good health. \u201cAnd why would I think I was having a heart attack without chest pain?\u201d\u00a0<\/p>\n

Casalan eventually got herself up and to her mom, who happened to be visiting that night, and from there \u201cit was a total of about five minutes between the realization that I could be having a heart attack to unconsciousness.\u201d Turns out she was suffering from full blockage in her left ascending artery\u2014prompting a heart attack known as a \u201cwidowmaker<\/a>\u201d\u2014which has just a 12% survival rate outside of hospitals for women. (Doctors have since theorized that it could\u2019ve been brought on<\/a> by having had an \u201coverly inflamed\u201d heart after a bout with COVID.)<\/p>\n

What followed were several cardiac arrests<\/a>\u2014sudden stoppages of the heart\u2014that required resuscitation, and being placed on life support for her heart and lungs.\u00a0<\/p>\n

\u201cMy family was advised to make their preparations and say goodbye,\u201d she tells Fortune<\/em>, and they were informed that her best chance at survival would come from a heart transplant<\/a>. She was placed on a waiting list.\u00a0\u00a0<\/p>\n

Today, Casalan, who has headed the 5,700-store network since 2021 and who, just days before her health crisis, had shared the stage with the company\u2019s CEO and CMO at a conference and was feeling \u201cready to take on the world,\u201d has come out on the other side of a long road to recovery dotted with setbacks. But she\u2019s also eager to talk about it all, as \u201chelping women work,\u201d especially moms, is a \u201cpersonal passion,\u201d she says\u2014as is health equity<\/a>.<\/p>\n

\u201cSo it\u2019s just a great extension of two things that I care so passionately about,\u201d Casalan, 49, now a board chair with the American Heart Association of Chicago, says. \u201cHow do we model for women how they can be successful in the workplace and be successful moms? Be successful single moms? You have to be a healthy mom to be able to do all of those things.\u201d<\/p>\n

Below, Casalan, shares just a few of the valuable lessons she learned from her near-death experience\u2014about leadership, parenting<\/a>, and setbacks.\u00a0<\/p>\n

Have some faith in medicine<\/h2>\n

Casalan remained on life support for many days and suffered initial setbacks\u2014including when she developed a blood clot that wound up cutting off blood supply to her leg and foot, requiring extensive surgical efforts to save them. She stayed in the hospital for over two weeks.\u00a0<\/p>\n

\u201cI was sent home with a life vest, which is an external defibrillation device that anticipates your higher risk of cardiac arrest,\u201d she says, and entered cardiac rehabilitation. \u201cThe idea was, hey, if you can survive the first 90 days, maybe we can kind of get past this transplant idea\u2026And I\u2019m here today to tell you that I have my own little heart.\u201d<\/p>\n

Casalan has recovered the vast majority of her heart function. \u201cMy message there is: Science matters. Medication matters.\u201d At a recent appointment with her doctor, she was told, \u201cListen, you can do all the lifestyle things<\/a>. You can do all the intervention things. But the medication and the science is what got you here.\u201d<\/p>\n

Listen to your body\u00a0<\/h2>\n

Since her heart attack, Casalan has discovered, through the emerging science of genomic risk analysis<\/a>, that she does indeed carry a 70% higher than average risk of cardiovascular disease<\/a>. Had she known, she might\u2019ve lived differently years ago.<\/p>\n

\u201cI lived in New York City for 15 years. I worked in the fashion industry. I was single. I was living the most extraordinary and full and interesting life, sustaining myself on a diet of caffeine, bagels, M&Ms and Diet Coke<\/a>,\u201d she says. In those days, she recalls, her mindset was one of, \u201cI\u2019m just all in on everything and everyone and everywhere, and I don\u2019t have to take care of myself.\u201d At the same time, she had \u201ca little bit of the typical mom piece and the typical female leader piece, like, \u2018I\u2019m going to take everything on.\u2019\u201d Eventually, that meant adding a \u201chighly contentious divorce\u201d into the already stressful mix.<\/p>\n

What Casalan has come to understand about doing it all and taking care of everyone but yourself is this: \u201cIf you don\u2019t listen to your body, it will speak for you eventually\u2026My invincible persona was re-educated.\u201d<\/p>\n

Good leaders are vulnerable\u2014and know how to roll with setbacks<\/h2>\n

Casalan had some big lessons sink in when she eventually returned to work. \u201cFor my team to have confidence and understand where we all were at the time, I had to be very honest about everything\u2014including what my limits were. And that was very difficult.\u201d What she believes that fostered, though, \u201cwas an openness from us as a team to talk about the realities that we\u2019re all managing and how we can help and support each other.\u201d<\/p>\n

The biggest change in her leadership style, however, \u201cis how I consider setbacks,\u201d she says. That\u2019s because she faced even more during her recovery\u2014namely, 70% blockage in another artery, her left main artery, discovered during a stress test at the doctor\u2019s office and prompting immediate robotic-assisted bypass surgery.\u00a0<\/p>\n

\u201cThat one was hard,\u201d she says. \u201cI think I had always anticipated that there would be some type of setback\u2026[but] that\u2019s not what I expected, for my healing to kind of be picked up and off of the rail.\u201d\u00a0<\/p>\n

As a leader, she shares, in her aforementioned \u201cindestructible phase,\u201d she had a tendency to \u201crun through all obstacles,\u201d believing, \u201cthere\u2019s no constraint we can\u2019t eliminate. We put our minds to it, and we can do it.\u201d But her second blocked artery changed her mindset.<\/p>\n

\u201cNow the way I think about setbacks is to say some of them are very far out of our control and very far out of our influence,\u201d she says. And she\u2019s more apt to consider a range of options about how to go forward\u2014with the understanding that they may need to be a pivot to a different way of thinking. \u201cI think that it has opened up a lot of creative conversations,\u201d she says. \u201cBefore we just either give up or keep going, let\u2019s really spend the time thinking about, what does this setback mean, and how can we respond to it? And giving the time and grace to do that has been meaningfully different.\u201d\u00a0<\/p>\n

It really does take a village<\/h2>\n

When Casalan was unconscious and being taken out of her home by stretcher the night of her heart attack, her two boys\u2014both on the autism<\/a> spectrum\u2014were unfortunately not asleep. \u201cThey did see the paramedics take me away, and it\u2019s still, you know, it\u2019s still a moment for them,\u201d she says.\u00a0<\/p>\n

But they were quickly comforted and cared for by many people in their lives. \u201cI am extremely fortunate. I come from the line of cast-iron women, they are quite formidable,\u201d she says. That includes her sisters who came from the East Coast, one staying for eight weeks, and her mother, who wound up staying for a year. Plus, she has \u201can extraordinary nanny.\u201d\u00a0<\/p>\n

Despite the crisis at hand, she recalls, when it came to her kids, \u201cthe most important thing was that they were surrounded by love and a sense of safety and optimism. We didn\u2019t really talk about what had happened until I was okay\u2014like, we didn\u2019t talk about the severity of what had happened.\u201d They have since\u2014just as they recently attended a local fire and rescue open house day, where they were all able to personally thank the paramedics who were there that night, bringing some closure.\u00a0<\/p>\n

Now, she says, she openly talks about her brush with death\u2014especially with her younger son, who, coincidentally, had corrective heart surgery at 10 months old. Sometimes they \u201ccompare scars,\u201d she said, and they recently did an American Heart Association event together.<\/p>\n

Both boys are even able to joke about it all. \u201cThey\u2019re funny,\u201d she says. \u201cThey\u2019ll say, \u2018Well, Mom, you know you only live once! Except you.\u2019\u201d<\/p>\n
